U-100 vs U-40 Syringes: The 2.5× Mistake

Two syringes that look the same, with markings that mean different things. This is one of the few measuring errors that is both easy to make and large enough to matter.

The short answer

On a U-100 syringe, one unit is 0.01 mL. On a U-40 syringe, one unit is 0.025 mL. Drawing "10 units" on a U-40 gives you two and a half times the liquid you would get on a U-100.

What the number actually means

The U-number is not about the syringe's size. It describes the insulin concentration the syringe was calibrated for.

Both syringes can be the same physical size. The barrel might hold 1 mL either way. What differs is how the barrel is marked.

The ratio is constant, which is what makes the error so clean and so easy to miss. Nothing looks wrong. The plunger sits where you expect. The volume is simply 2.5 times what you intended.

How to tell them apart

Read the barrel. The scale is printed on it — U-100 syringes are marked to 100 (or to 30 or 50 on smaller barrels), U-40 syringes to 40. If the highest number on a 1 mL barrel is 40, it is U-40.

Check the packaging. U-40 is usually labelled explicitly, often alongside veterinary branding.

Colour is not reliable. Cap colours are a manufacturer convention, not a standard, and they vary between suppliers and regions. Do not use colour as your check.

"U-50" is not a thing

This one causes real confusion. A 0.5 mL syringe is not a "U-50" syringe. There is no U-50 concentration standard. A 0.5 mL insulin syringe is almost always a U-100 syringe with a smaller barrel that holds 50 units.

So its markings are U-100 markings — one unit is still 0.01 mL. Only the capacity changed. The same is true of 0.3 mL syringes, which hold 30 units on the same U-100 scale.

If you only have U-40

The arithmetic still works; you just need to use the right divisor. Volume in mL is units divided by the scale — 100 or 40 — and everything downstream follows from that. The safer habit is simply to use one scale consistently and to check the barrel before drawing, every time, rather than relying on which syringe you think is in the drawer.
